Bethsaida Hospital Serang successfully treats a complex, heavily calcified coronary artery blockage using a rotablator, enabling optimal stent placement and restoring blood flow to the heart.
Heart disease remains one of the leading causes of health problems in Indonesia, including in the Serang and Cilegon areas, which are regions with high industrial activity and community mobility. A busy lifestyle, high stress levels, smoking habits, as well as risk factors such as hypertension, diabetes, and high cholesterol, can increase the likelihood of developing blockages in the heart blood vessels.
On the other hand, advanced cardiac care services in the Serang and surrounding areas remain relatively limited, even though the management of heart disease greatly depends on the golden time, which refers to the critical period requiring rapid and precise treatment to prevent more extensive heart damage.
In many cases, blockages in the coronary arteries can be treated with a Percutaneous Coronary Intervention (PCI) procedure or stent placement through a catheter. However, in certain conditions, the blockage can become highly complex — such as when plaque buildup in the blood vessels has undergone severe calcification (becoming hardened like stone) — making it difficult to penetrate using standard tools such as angioplasty balloons or diagnostic catheters.
At Bethsaida Hospital Serang, a patient presented with chest pain symptoms indicating impaired blood flow to the heart. During the PCI procedure, a coronary artery blockage lesion was found that was extremely difficult to pass using conventional techniques.
Attempts using standard PCI, balloon angioplasty, and evaluation with IVUS (Intravascular Ultrasound) had been performed; however, the narrowing in the blood vessel remained impassable, preventing the optimal restoration of blood flow to the heart.
After careful medical consideration, the patient was ultimately treated with a rotablator procedure to overcome the hardened blockage and create access for the subsequent intervention. This rotablator procedure marks the first case in the Serang region involving the management of a complex coronary artery blockage.

Rotablator Procedure Opens the Path for PCI
After the stent was successfully placed, stent expansion was performed at the location of the lesion that previously could not be accessed. The process proceeded smoothly, improving blood flow to the heart and preventing further complications. Post-procedure evaluation using IVUS after PCI also showed excellent results: stent expansion was highly optimal, and blood flow in the treated vessel reached TIMI 3, indicating smooth blood flow without any residual stenosis or remaining narrowing.
“In this patient, we faced a very challenging condition where the blockage in the coronary artery was extremely hard and could not be treated with the conventional methods we usually use. By using a rotablator, we successfully created a pathway for stent placement, allowing the patient’s blood flow to the heart to return to normal. The post-procedure evaluation also showed excellent results, with optimal stent expansion and smooth blood flow. This is a real example of how the latest medical technology can provide the right solution for patients who require the best possible treatment,” explained Dr Fani Suslina Hasibuan, Sp.JP (K), FIHA.
“At the Heart & Vascular Center Bethsaida Hospital Serang, we are committed to providing the best services by utilising modern medical technology, including rotablator for treating complex coronary blockages. We are also equipped with comprehensive facilities, ranging from angiography, stent placement, PCI, to cardiac rehabilitation, ensuring that patients receive comprehensive care. The success of this procedure demonstrates our ability to provide timely and appropriate solutions for patients requiring more intensive cardiac treatment,” concluded Dr Tirta Mulya, Director of Bethsaida Hospital Serang.
